2022 Honda Civic

While driving steam started to appear from the engine bay. I pulled over to inspect, lifted up the hood and could hear the fuel line popping and gasoline hissing out. The engine bay spontaneously caught fire. As it ignited all of the gasoline in the air combusted and created a ball of fire in my face as i was looking in the engine bay. The fire was put out by the fire department. An incident report was created. The potential for safety risk was high and could have been worse if i was not on a road where i was able to stop or if it caught fire while i was inside the car. The vehicle has been inspected by insurance and confirmed to be a total loss. Since nothing was left to visually inspect of the car their investigation came back nonconclusive as to the cause. There were no indication lights that there was anything wrong with the engine.

I was stopped at a light and was the first vehicle. When it turned green i started to go. I saw a short ways in front of me one of those metal plates over a hole in the road. I started to coast from 20 mph with the anticipation i would be at 10 mph when i got to it. Just before i would go over it the emergency brake system kicked in and took the vehicle to a stop. I tried to make it go by stepping on the gas but it came to a complete stop. I am thankful the vehicle behind me did not hit me.

While car in sport mode in center lane of a three lane road at 45mph traffic ahead by over 300ft began gently slowing down to +/- 25mph extreme braking was automatically applied w/o driver input, "brake" message was displayed flashing on instrument cluster. Car came to an immediate stop, nearly causing collision with another vehicle behind it.

Driving up snowy pass on i-90 approaching the top of snoqualmie pass. Vehicle stalled and warning lights flashed critical warning and loss of power. The vehicle stopped in the middle of the highway and would not start or go into drive. After a few minutes, because of speeding and adverse conditions, another driver collided with my vehicle from behind. Another following vehicle lost control and ran off the road. Nobody was injured. Mazda has not provided any information to what happened with the vehicle. We added about 12k miles in the 5 months of driving it, so we had lots of testing with it, and never ran into this serious of an issue with this or any other vehicle.

The vehicle hesitates for several seconds and then very forcefully hard shifts going from 7th to 8th gear every time. If i am stuck in a steady flow of traffic where it is constantly shifting up and down for lights or people stopping to turn, the vehicle will sometimes lurch and hesitate multiple times in a row like it's playing tug of war. I've taken it to to the dealer for all my regular oil changes and ford keeps telling me it's normal because of the "adaptive driving". It has done this since i purchased the vehicle in 2019 and progressively gotten more aggressive as time goes. I currently have just over 75k miles currently. I can get online and search 2018 ford 10 speed transmission hard shift issues and people are complaining left and right. Many have said it has led to transmission failure prematurely, as early as 65k miles. Ford has no fix for the issue but all the ford service departments know about it. We have seen service bulletins released regarding the issue but no permanent solution to actually fix the issue. And yet, ford still refuses to issue a recall for this specific transmission issue. We already have a 2018 ford focus with a blown motor at 65k miles sitting in our driveway due to fords neglect and i doubt it will be long before we have a broken truck too if this is allowed to go unaddressed. I am terrified that i will be driving to or from work and the transmission will fail catastrophically and leave me stranded on a busy highway with no shoulders. I hope this persuades someone to take action. Thank you.

1) what component or system failed or malfunctioned, and is it available for inspection upon request? I tried to park in the parking space next to it by making a slight right turn. As soon as i stepped on the brake to park, the car began to rush forward. I noticed again and again that my foot was slamming on the brakes, but the car didn't stop. My car went over the ledge between the parking lot and the store, slammed into the pillar of the building on the left, smashed all the glass in the dance studio, and hit the wall on the right before coming to a stop. It took six seconds from rushing forward to into the store. 2) how was your safety or the safety of others put at risk? I must have been severe injured if the car didn't stop by hitting a right-side wall of the store. And if there were people, i was not able to avoid casualites. 3) has the problem been reproduced or confirmed by a dealer or independent service center? No, the car was towed and reviewd by car insurance. Car insurance informed that th e car will be total loss. No information about reproducing the issue 4) has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No 5) were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No, that was suddenly occured 6) request: i want nhtsa to investigate my car of sudden unintended acceleration. I would like to get edr, x-ray of pcb board of electrical central unit(a. K. A ecu) managing brake and accelerator and car manufacturer's reliability sign-off report of the ecu board. I have a security footage taking the accident moment. I failed to upload the video. 2024 Honda ACCORD

Car has 4800 miles. I've owned since new (5 months). No issues prior to this. Stopped a stoplight. The gas engine stopped running, which is typical for a hybrid when not moving. After 15 seconds, every warning/error message started cycling on the driver's dashboard screen, referencing "brake system malfunction" among many other failures. Pressing the gas pedal and there was no forward movement. Brakes still worked. Put in park. Pressed "start" to shut down car. It didn't fully shut down (dash still lit up). Pressed start again, still many errors but car would move. Drove immediately home. After 2 hours, pressed start again and no errors. After 2 days of no driving, got appointment with local honda dealer service. Drove it carefully and had no issues. After two additional days of evaluation, honda says it's the ignition control module (part# 5k80030ba06). They had to keep the car longer to allow regional honda rep to inspect and document the car's issue, as they said this issue is becoming a trend. Lastly, they told me it could take 6 months to get the part, due to a "national backorder" but offered no options/alternatives. Said car is unsafe to drive. This is unacceptable and i hope is investigated. This is a very dangerous issue. Had i been driving, it could have been far worse result. Attached is a photo of 1 of the many error messages that rotated on the dashboard.

2015 Acura RDX

My 2015 RDX lowbeam headlight getting dimmer gradually. It is too dim to the level of undrivable at night. I replaced the light bulb, but it doesn't help. It almost cause a collison that the opponent car from side of road can not see the light.
