The vehicle has on numerous occasions performed automatic braking when no cars are near the front. This is causing the car to break abruptly and i’m fearful that a vehicle to the rear of me will collide due to the erratic and forceful application of the breaks.

Transmission slip/jerk/clunk when going above 2 RPMs. This has happened before 3 times with the first time happening in 2019, then 2022 and then again on April 15th, 2024. I've taking it to the dealer all 3 times and they performed a transmission fluid flush since apparently the transmission fluid gets too hot and needs to be flushed every so often. This last time they told me I needed a new torque converter ($5,500) to fix the issue. This a huge safety issue especially when driving in the highway as the cars loses speed and begins to jerk. I see similar issues with the 2017-2019 models (https://static.nhtsa.gov/odi/tsbs/2023/MC-10241879-0001.pdf) which sound the same as to what's happening to the 2016 models. There are a ton of complaints on the web for 2016 Pilot which the NHTSA should take into consideration.

The automatic emergency braking system will turn on when there is no danger. It happens often. The system is fooled easily. Yes it’s available for inspection. How was your safety or the safety of others put at risk? With normal driving conditions this is not a huge problem but if this were to happen with slick roads. I could see this causing an accident. Has the problem been reproduced or confirmed by a dealer or independent service center? No - i’m not sure how this would be done. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No

2016 Honda Pilot

Dealership involved is lamacchia honda at 932 genesee st, syracuse, ny 13204. They have the failed engine and vehicle currently. The engine failed rapidly, and could have caused a unexpected stall and made the vehicle hard to control, had it not happened while the vehicle was parked. The issue was noticed in early march, approximately 116,200 miles on the odometer, with a small knocking noise not noticeable under all conditions. Once the knock noise was discovered, the vehicle was driven approximately 100 miles to return home. The next morning the car was started, in order to identify the noise. The engine was running for 4-5 seconds before the knock became rapidly worse, and a low oil level warning was displayed. I immediately shut down the car, and confirmed that the engine oil level was full. After this, i drained 1 liter of engine oil for analysis (from blackstone laboratories), and parked the vehicle until it could be towed to lamacchia honda. After the engine was disassembled, the dealership stated that the #1 rod bearing and crankshaft journal were damaged. No repairs have been completed yet. Dealership has suggested a long block or engine replacement will be required. I believe this incident is related to nhtsa recall no. 23v-751 attached is the invoice from the dealer.

When driving down both highways and interstates on several occasions my vehicle has slammed the brakes on prompting the warning of emergency braking. This when there are no vehicles directly in my lane or path. It often occurs when passing a tractor trailer on the interstate so much so that i hover my foot on the gas pedal to counteract this braking.

Frequently, my 2021 honda will apply the brakes without warning or very abruptly. While using the adaptive cruise control, the car will brake suddenly when there is not a car anywhere near me. Also, when driving on two lane roads, the car slams on the brakes when there is not a car in front of me.

My car has been reliable other than standard maintenance; oil changes, brake pad changes, etc. On friday, april 12 2024, i was driving down the highway at 60-70 mph when the engine felt like it lost power for half a second and i saw the rpms rev up and then drop down when this "skipping" occurred. It happened several times and made me feel very unsafe in a high speed situation if i suddenly lost all engine power. I drove straight to my mechanic - they did not experience the skipping at low speeds on a test drive, but they recommended a transmission fluid flush. On draining the fluid, they said it was clean but had more metallic debris than would be expected for a usual transmission service. I drove the car over the weekend and the same engine loss of power was evident, but happened infrequently. Then on monday, april 15 2024, the engine was losing power on the highway again and on the dash, a "d3" code was blinking. I went back to the mechanic, at low speeds, where they determined the transmission needs to be replaced - upon speaking with their honda contact, this is becoming a known problem where transmissions are needing replacement in the 60-90k mile range. My car is at about 78k miles, we did have the transmission serviced at 34k miles and then not again until friday the 12th - however there were no warning signs/sounds or error codes until the power loss on the highway and finally, the error code on monday. The car is currently at the repair garage awaiting the expensive, replacement transmission and is available for further inspection.

Vehicle has applied brakes automatically for no apparent reason at least 10 times in the 4 years we have owned the vehicle. 2 of those incidents was entering a freeway under acceleration from a on ramp putting us in a dangerous situation. It did not completely stop us before it released the braking system. It has also happened when exiting the freeway onto a off ramp numerous times automatically applying the brakes when nothing is in front of us or beside us. Last time it happened was last week while driving down a divided highway with no vehicle beside me or anywhere close in front of me a car was behind me and had to slow down. My car automatically applied the brakes and slowed me down and would not accelerate for about 10 seconds then they released and i was able to accelerate back to normal speed. To not it does not slam the brakes on and lock them up but it does start to slow you down quickly before allowing you to accelerate again. In the wrong situation this could cause a accident or a string of accidents.

The forward collition mitigation system is constantly braking on its own for no apparent reason. It happens a lot when going around a slight right turn and it interprets cars coming the opposite direction in the other lane as a trigger to start braking. It is a very dangerous situation each time, so i turn off that feature every time i start driving now to avoid it.
