2019 Tesla MODEL 3
  • Date of Incident: Oct-12, 2023
  • Date complaint filed: Apr-22, 2024
  • Failed components: SERVICE BRAKES, STEERING, and FORWARD COLLISION AVOIDANCE: WARNINGS

The following recalls have been pending on my vehicle: 23v-838; tc2023-657, 24v-051 tesla is unable to push over-the-air (ota) updates to my vehicle to remedy these recalls due to an issue with the onboard computer being unable to receive updates. This issue arose when tesla pushed a software/firmware update to my vehicle which effectively "bricked" the advanced drive system portion of the computer. In other words, i am unable to safely receive an update to resolve the issues (solved by the recall) because tesla's failed software update damaged the onboard computer.

2018 Tesla MODEL 3
  • Date of Incident: Apr-15, 2024

  • Failed components: UNKNOWN OR OTHER, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, and FORWARD COLLISION AVOIDANCE: WARNINGS

Following an over-the-air software update, all audio in the vehicle stops working. This includes entertainment services, but more importantly turn signal indicator "clicks", forward collision warnings, parking assist chimes, shift confirmation and warning chimes, and various adas notification sounds. In some instances, the problem corrected itself within 24 hours. In another instance, it persisted for nearly a week before i discovered that unplugging the usb cable for the dashcam fixed the problem. The dashcam hard drive has been installed for over 4 years, per the instructions in the user manual, without problems until early 2024. There were no error notifications or anything else to indicate that there was a problem with the car's safety systems, or what the cause of the problem was, except for the lack of audio. I chatted with tesla service about the issue. After discovering and sharing that disconnecting the usb drive fixed the issue, they were persistent in wanting to cancel my appointment without gathering any additional information. Here is the transcript of our interaction: me: unplugging my dascham hard drive from the usb port fixed it. Weird. Tesla: oh interesting. Good to know! Do we have permission to cancel this appointment? Me: since it keeps happening i'd like to keep it open. I turned on remote debugging in case that helps you to report the bug or something. Tesla: we have nothing else to diagnose if your vehicle is operating as expected. Me: ok. If it keeps happening i will still want to bring it in, so let's keep the appointment. And i wouldn't say it's operating as expected. Having to disconnect usb cable to get the turn signal to work properly isn't what i would expect. Tesla: i would hate for you to bring in your car, only for us to tell you that it is because of your third party accessory, and we charge you for the diagnostic time. Me: i appreciate your concern.

After around a year, it happened again and i repeated it. They have an upgraded replacement part but are not recalling the defective original. Can you help?

2024 Tesla CYBERTRUCK
  • Date of Incident: Apr-22, 2024

2023 Tesla MODEL Y
  • Date of Incident: Apr-08, 2024
  • Date complaint filed: Apr-22, 2024
  • Failed components: ELECTRICAL SYSTEM, POWER TRAIN, and VEHICLE SPEED CONTROL

Car warned that drivability - acceleration and top speed reduced/performance may be restored on next drive but gave no opportunity for me to pull over safely. Felt like someone pulled the e-brake during regular driving speeds and prevented me from safely slowing down, steering or maneuvering the vehicle into a safe spot. This occurred during a rainy night with my wife and 3 daughters in the vehicle who were terrified and still afraid to drive in the vehicle. Tesla replaced the front drive inverter and about a month later the same issue happened again on a busy road around 1pm in the middle of traffic and this time the rear drive inverter had to be replaced. Myself nor my family have felt safe in the vehicle but feel stuck with it.

2021 Tesla MODEL 3
  • Date of Incident: Apr-21, 2024
  • Date complaint filed: Apr-22, 2024
  • Failed components: FORWARD COLLISION AVOIDANCE: WARNINGS, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, and FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

While using the full self driving mode (fsd) the vehicle will randomly and without warning slam on the brakes at highway speeds and decelerate to 30-50mph on an active highway. If the highway is multilane, the car will slow and attempt to change lanes. It appears the vehicle thinks that something is in the road, even if the road is clear. This happens multiple times on my longer road trips, and is very concerning. It did this yesterday with a semi-truck closely behind me. The vehicle has gone into service and i did bring up the issue, but the service team was unable to assist on this issue (screenshots attached). The software currently installed in my vehicle is 2024. 3. 15, the latest available from tesla. This issues has been ongoing from every software update i have had in the car however.

2023 Tesla MODEL Y
  • Date of Incident: Apr-20, 2024
  • Date complaint filed: Apr-21, 2024
  • Failed components: SEATS

We had our 5 year old son buckled in his forward facing car seat in the back passenger seat. When i started to drive my car, the front passenger seat started to move back, reclined on its own and fully crushed my sons legs in the back seat (fully reclined back). My son started to scream in pain. After securing the seat forward, we noticed our umbrella fell from the front seat and landed where you control the seat alignment in front. We were shocked to see that a 1 lbs umbrella can be heavy enough to control the seat alignment. If i hadn't quickly reacted to the seat moving back, it would have caused a serious fatal injury to my son. I strongly urge the manufacturer to consider fail proof ways to prevent this from happening.

My tesla is supposed to have "automatic" wipers that go on by themselves to keep the windshield clear so one can drive in rain. Manual wiper speed control is difficult because the driver has to look at center screen and use menus to select a speed. There is no intermittent wipers control; the lowest speed setting is too fast for light range. Only one pass operation is possible without using the center console while driving which is dangerous. The automatic wipers are very erratic, sometimes running faster than needed or coming on when it's not raining to other times hardly coming on at all making it very difficult to see and making driving very accident prone. The poor operation can, of course, adversely affect the proper operation of the automatic driving capability too since that depends on vision by cameras through windshield. Other owners i know voice the same concerns. This issue is a big problem anytime it rains and even worse back last winter when it snowed. I've only had my car 3 months. I have talked to a few other owners who indicate this problem didn't exist as much 6 months or a year ago indicating it could be a problem caused by a recent car software update or a change in the equipment used for wiper control in my tesla y. I'm not sure if the problem applies to other models (ie s, x, 3).

2020 Tesla MODEL 3
  • Date of Incident: Apr-20, 2024
  • Date complaint filed: Apr-20, 2024
  • Failed components: SUSPENSION

I am writing to express my concern regarding the recurring issue with the control arms and sway bar in the 2020 tesla model 3. It has come to my attention that tesla acknowledges this issue, which manifests as creaking sounds during various driving conditions such as climbing into the car, traveling at low speeds, and navigating over speed bumps or uneven terrain. What's particularly troubling is that when i brought my car into them tesla acknowledged that they are aware of the prevalence of this issue across many of their vehicles, yet they continue to charge consumers for repairs, even if their vehicle is not included in the recalled vins. This policy unfairly burdens consumers who experience the same issue but are denied coverage under the recall. I urge the nhtsa to investigate this matter thoroughly to ensure consumer protection and fairness in addressing safety concerns in tesla vehicles. It's essential that manufacturers uphold their responsibility to rectify known issues without imposing undue financial burdens on consumers.
